- Cardiac arrhythmias such as atrial fibrillation, supraventricular tachycardia, complete heart block, and ventricular tachycardia may occur in patients infected, recovering and recovered from COVID-19.
- Over the last one year, I have seen a few cases of arrhythmia (heart block, inappropriate tachycardia, atrial fibrillation) in patients post Covid with mild cases of infection or even asymptomatic.
- Even though the mechanism of action is not yet completely known, inflammation of conduction system, myocarditis, and prolonged QT are proposed culprits.
- It is interesting that arrhythmias can occur post recovery and even in mild cases of Covid, probably due to sub clinical inflammation of the myocardium , or undetected prolonged QT ( not related to medications).
- Most of the arrhythmias were transient, but required temporary treatment, due to symptoms. It is important to recognize the Covid related arrhythmias for two reasons: first, to monitor and provide symptomatic relief, and second, to understand the possible temporary course, and to avoid immediate aggressive therapy, such as pacemakers or defibrillators.
Arrhythmias in the presence of symptomatic, critically ill Covid patients is associated with worse prognosis
Arrhythmias may occur as the only manifestation of Covid, or post recovery of mild or asymptomatic Covid infections. Invasive procedures such as pacemakers or defibrillators should be done with caution, since the conduction abnormalities may be temporary.
